Your surgical team determines the treatment area for surgery preparation. Facial and body benefits can differ; ask about your exact plan and clinical reason.
Confirm eligible provider types, referrals, prior authorization, approved dates and units, cost sharing, and balance-billing risk in writing before treatment. Coverage and reimbursement are not guaranteed.
Illinois network-access protections apply to certain individual and fully insured plans. Self-funded plans can follow different rules; exceptions are not automatic.
Planned self-pay sessions: $90 for 30 minutes, $150 for 60 minutes, $225 for 90 minutes, and $300 for 120 minutes. An additional 15 minutes is $37.50 when available as an extension. Prices and availability require confirmation before opening.
